I'm on version 1.3 and I still have the missing icon issue with some of the minigames, especially the dicepoker game.
This is a nasty thing in particular since it happens at random, and doesn't allow you to do anything, and no input is recognized anymore.
The only way to get out of this is by killing the game the hard way with ctrl+alt+delete.

I use windows 7, and don't know whether this happens on older OS' as well.
In chapter 1 when I'm required to escort Ioverth up to the garden and through Flotsam Geralts hair would "flail". This is with the dangling limiter on.

Akhotnik: 10) When fighting in the mist (during the entrance into Chapter 2) stunning and killing the draugir can result in Geralt’s death. The problem is the cut scene finisher takes place outside of the protective bubble. Thus, Geralt takes damage while killing the draugir.
This bug is really P.I.T.A. Draugir are very easy to stun and execute and would be the most logical way to handle the fights, if not for the bug.

Solution: There shouldn't be any health loss during cut scenes.
Just another update to the first post:
crashes to desktop still occur in version 1.35. In my experience it happens while simply playing, neither loading new areas, nor saving. Frequency is variable and it doesn't occur every time. Seems random.

Crashes more often occur when launching the game.
I was also having a lot of crashes to desktop, but disabling desktop composition in compatibility options seems to have improved it. I need to play some more to see if it has actually stopped.
Apologies if it's already been mentioned, and forgive me if this is not a bug...

On Iorveth's path in act 2, if you complete the labyrinthian combination of royal blood, the walls have ears, and suspect: thorak successfully (meaning, you got a green check next to each of them during act 2), the latter two (walls have ears and suspect: thorak) are flagged as failed at the beginning of act 3 (red x next to them in the journal).

Again, if this was cited earlier in the thread, sorry for the repeat; and if this is by design, I'd love an explanation of why.

I think there are some more bugs that have been reported here that have yet to be included, I'll try to do so soon.
Bug: Dice minigame is broken in the prologue chapter, when playing with a gamepad.

Where: Prologue chapter, at king Foltest camp, near the armwrestling guy.

What happens: Player is able to start dice match with the Ai, but cant finish the dice game, because buttons become unresponsive to player. Player is able to throw the first round of dice and select what dices player wants to keep, but is unable to continue after that.

(Happens while playing with a gamepad)

Solution: Players should be able to play the dice game from start to finish,

I tried it three times and everytime it did the same thing.

The way I played was that I first armwrestled the nearby guard and then started to play the dice poker match. That way it happened every three times on a row, but I also noticed that if I just ignored the armwrestling guy and played dice poker first, then I could play the whole match from start to finish.

So it seems that the armwrestling minigame somehow affects the dice poker game, and the only way to get out of it is to force the program to close.

@People who experience(d) the unresponsive controls bug: Was this bug fixed in 2.0 as stated in the changelog, or do you still have problems?

Level up

Game does not recognise that you have allocated the attributes when levelling up. Therefore the game breaks as the message "level up instructions" does not go away, stopping you from continuing the tutorial.

Worked fine for me. Have you been able to reproduce the bug? Can you describe the exact steps you took to get this result (e.g. did you open the character screen before the instruction to do so appeared, which skill did you buy)?
I saw someone else had the problem on Gamefaqs forum, I do as well.

ESC (key) doesn't work on Inventory (i) or Skill Tree (c). You have to click back/exit every time. ESC does work for pausing the game and navigating the load/save/exit screen.

This occurs throughout the game? I guess it applies to all menu screens (journal, alchemy etc.)? What keyboard do you use?

If your keyboard uses special drivers or some background application (Microsoft Intellitype, Logitech Setpoint), try to close those before launching the game.
