Dragon Age 2 what where they thinking.
I don't remember much of it so maybe it's better than I remember, but I seem to recall Bushido Blade 2 being a complete and thorough letdown after the greatness that was Bushido Blade. It was bad enough to pretty much kill the series, which is a shame because the first game was really really innovative and still holds up today.

If standalone expansion packs count, Gothic 3: Forsaken Gods definitely gets a shout here. Somehow buggier than the legendary brokenness of Gothic 3, a stupid retcon of Gothic 3's ending, changing mechanics that were core to Gothic 3 and closing off the lands of Nordmar and Varant, changing the lead character's voice actor for dialogue but reusing the the old voice actor's sound effects (like the pain shout) and stuff like that. Awful...just awful.

Breath of Fire V Dragon Quarter.

Apparently, Breath of Fire VI will be released this year in Japan, but doubtfully so in the West. Is it bad thing or not, I am not sure, because BoF VI will be made around touch-screens of smartphones...
Fallout: New Vegas
Most of the quests made you speak to the same NPC-s repeatedly while you spent your time walking in discovered areas between them where nothing happened. Preferably involved multiple loading screens due to the lack of reasonably placed quick travel points. I think 70% of my game-time spent walking from point A to B. (and often having to go back to A on the same route just to make it more annoying)

The quests in that game are literally like two wee girls in school who aren't talking and you're the third friend relaying the messages back and forth. That game was so dull.

WoW wasn't a sequel to anything though, it was stand-alone game, having connection to Warcraft RST series just in name.

However, if people imply Cataclysm, I full-heartedly agree. But I guess such is the fate of any MMO - change of directors/game designers -> completely ridiculous expansions. Not really much for persistent world...
No mention of Duke Nukem Forever?
Also another vote for Dragon Age 2, the game that PC Gamer spooged over and ranked in the top 100 PC games ever.

From what I have read, the game is different from the rest of the series and might have been better if it were called something else.

Of course, I avoided that game because of the save system, the D-Counter, and the fact that the save files were copy protected. (Of note, I at least would have had the US version with the quick save rather than the European version which omitted that crucial feature.)

Anyway, my example is ActRaiser 2. Take a game that consists of generic (for its time) action sequences with unique simulation sequences and remove the unique simulation aspect, resulting in a generic action game. (Also, I never cleared the first level because of a jump I couldn't make.)
Speaking of the Fallouts, Fallout 3. Not going to go into too much detail as I've blathered on enough about it in the past, but the legendary stupidity of the writing and world building is reason enough for it to be listed. It sucks as a Fallout game (regardless of the genre style change) and it's a mostly dull action game to boot.

It continued the storyline; and in my opinion completely destroyed it.
Should there be a Warcraft 4, its story will have to take into account WoW and other Warcraft games. It won't be just a continuation of Warcraft 3.

bof5 is the best game in the series because it actually does something interesting and fairly unique

Well there were much worse Duke Nukem sequels. MUCH worse.
