TrulloF: unpack with 7z and move the files inside the "roth_us_files" folder to your game directory. just look where roth.exe is located on your disk and copy the files there. overwrite when prompted.
I don't own the gog-version so I can't be more specific. Sorry.
This didn't work for me. What did work perfectly, however, is using the US files to overwrite the CD1 UK files and *then* installing the game from the US-patched CD1 and of course the rest of the CDs (I'm also running the entire game from the hard drive with no physical CDs to worry about. As this is so easy to do I'll assume everyone is already running the game that way.) WHen I tried simply using the US files to simply overwrite the pre-installed UK game files my sound immediately broke down into high-pitched and faint unintelligible squealing and screeching. So I think best installation is to use the US files to overwrite the CD1 files from the UK version and install the game from there.

It worked for me. Only the Interplay movie which plays after you start the game is split in two parts and these parts are exchanged. One part of the movie which should be on the left side is on the right side and the other part which should be on the right side is on the left side.
Gog version of ROTH 2.0.0 does not include this patch?
The gog-version of Realms of the Haunting is not like the one that you get from this patch.
I bought RotH some days ago when it was on sale and after an initial launch without the patch, I quickly installed this here patch/upgrade.
It is very simple to install. (I double-installed into two places inside the install directory -see older post in this thread- to be sure that it works)

The first difference is the addition of a difficulty selector in the menus, which you can read about here:

The other significant difference (which goes oddly unmentioned at that page) is the ability to re-configure a sub-set of the control keys. Unfortunately some keys are still hard-coded, so you may still need to use the DOSBox key mapper; but it still offers a huge improvement on the (absolutely horrible) default layout.

Can anyone explain how to install the Patch on Mac? I already extracted the patch exe so that I can copy the files manually in the game folder but when I open the package content in programs I can't find the files that need to be replaced.

EDIT:

OK I found the solution. The files are hidden in the "Realms Of The Haunting.boxer" file.

I successfully installed the patch, and the difficulty settings and key rebinding options have been added. Unfortunately, the patch has had the side effect of screwing with the colors/lighting. In dark rooms with candlelight, it seems like the colors are separating, so that you see bands of blue, red and green, rather than anything remotely looking like candlelight. Anyone else with this issue?

EDIT-- or am I just going crazy? Was it always like that? Can't actually remember now.

It messes with the sound, definitely. The speech is nothing but static.
