tinyE: That was my 1st guess but that guy had armour.
Note that the armor represented the health level, being hurt would remove armor pieces and ultimately the guy would just run around in his undies, a bit like in this pic.

Yeah, but in this game the protagonist is a real macho - never wear anything to cover his torso ;) And yes, the main quest is to rescue a beautiful woman ;)

I actually havn't played it but the picture reminded me of the epilepsy warning Nintendo had to start issuing after Kid Icarus.
