The reason why gog removed many indexed old games from the german store, was that the BPJM got a new leadership and i guess she wanted to make an impression by pushing the given rules. The new law was established a few years ago, to also include online-download stores, before that indexed games on gog and steam were a grey area. The old administration did not enforce these new rules however, maybe because they didn't care about old games that didn't even appeal to younger players.
I am curious about your source here. Because the BPjM does not index any games by themselves. They have to be asked to do so. There is a difference between rating and indexing a game.

Gog usually never releases cut versions (if they would, many censored versions of indexed games would be available in Germany on gog).
It is still possible, that a German language selection will also apply the graphical censorship, so play the English version to be on the save side.
Not true. GOG usually sells the saame versions that are officially available. So if a certain game is censored in a certain country it is censored on GOG as well. One example of this being GUN.
- I was talking about games that were already on the index, but got ignored by the BPjM on gog until the administration changed (I can only assume this, since both events happened ca. 3 weeks in a row).
- I meant gog does not release separate regional edition of games, only international ones. If a version of an indexed game contains a German language option with censorship, as well as an uncensored English option, it will be completely region locked.
