toxicTom: Sometimes it's "kill your darlings". But sometimes it's a matter of time and making the release date. In Planescape: Torment there was an area (late in the game) that was clearly not finished. Texts were missing (cursor indicates description but nothing happens) and you could get stuck going into one of the areas since they forgot the exit. I'm glad there are restoration patches for that.

Some Skyrim mods add back NPC banter (i.e. guard dialog) that was (probably) accidentally forgotten. I mean there even are voice recordings for that and the lines totally make sense within the game.

Fever_Discordia: Also, I've not experienced it first hand, but KOTOR 2 was famously released in a no-where near finished state - apparently the huge content restoration patches bring it UP to a half-finished level, or something...
From what I've heard the KOTOR2-patches are a bit double-edged. They add back some things that are needed to make sense of the stories but (some?) also include areas that maybe have been left out for good reason and are rather bland to play. But I've never played KOTOR2, just the first, so I wouldn't know exactly.

Well, this thread was originally only about fix mods, but now I'm checking a few others types too (but I still prefer the first ones).
I usually don't like ports, but if they're REALLY good, I could check them.

Rodzaju: Temple of Elemental Evil is pretty much unplayable without the Circle of 8 modpack.
With the modpack it's a hidden gem.
The poster child for "Do not rush a game to market, you stupid publisher."
Thank God for the Circle of Eight. They made it a great game on their own time.

Unofficial Oblivion Patch, Unofficial Shivering Isles Patch and Unofficial Official Mods Patch for Elder Scrolls: Oblivion are good, they fix a lot of bugs in the game.

For players of Command & Conquer: Renegade, the Tiberian Technologies unofficial patch fixes a whole pile of bugs and adds a bunch of new features and stuff. It doesn't make changes that ruin the gameplay or change the balance though. You can get it from http://www.tiberiantechnologies.org/downloads (disclaimer: I am the lead coder of this mod :)

There are some unofficial bug-fix patches for various other C&C titles as well although I don't have the details about those.
