The patches are uploaded to GOG by the developer himself. GOG only tests the patch. As the patches are first uploaded to Steam, GOG gets the patches later.

According to the devs, the patches were uploaded to GOG, but people who don't use Galaxy get them when GOG feels like it. Here is what the dev got back from GOG:

"...there are offline installers available through our website without the need to install the Galaxy client, but they may be delayed comparing to the Galaxy installers and this is the case for the Project Hospital update. We are working on improving this, apologies for any inconvenience!"

I guess GOG really wants to strong-arm us into using their Steam look-alike. That is not going to happen in my case. With age comes patience if not cunning.
