I'm looking for a good DOOM source port that is easy to set up and use. I don't want custom 3D graphics, models or shaders. I only want easy M+KB control and support for widescreen resolutions. Something like eduke32, which allows you to uncheck its polymer pack and run the game pretty much stock. Also preferably something that can use wads for ultimate, final and doom 2.

Is ZDoom alright? That's what I use.
Yes, zDoom is really good and got lately also an update. As far as I know it got some graphical improvements (better resolution), but still close to the vanilla feeling.
ZDoom is what I use as well. And it seems to be alive and kicking, latest update was 1 July 2012 at 19:20 :)
It can take any wad I've thrown at it and it doesn't use custom 3D models etc., just support for better resolutions, M+KB and free-look. I'm not sure if it supports old replays though.
I would recommend GZDoom. The main attraction of that source port over Zdoom is that you can use mouselook to a full 360 degrees, which is useful and feels more natural in my opinion. Here is the link to the latest version, which is based on the v2.6 edition of Zdoom.

Chocolate Doom maintains the behaviour of Vanilla as close as possible, but adds higher resolutions, as well as easy loading of stuff that Vanilla couldn't do (like loading flats, sprites and dehacked patches from the command line). It is a warts and all affair though; where the relevant version of Vanilla would have crashed, Chocolate Doom will too, for the most part.

Lest I be called out for sheer laziness, does ZDoom support proper horizontal mouselook? I too have been looking for a decent Doom source port that doesn't stray too far from the original.
Phh, I play pure-original-out-of-the-carboardbox vanilla. Old school.

I tried that but the fast past keyboard action was giving me RSI. It definitely does take me back to the old days though.

That game used to scare the hell out of me. Especially flickering lights. I have developed an intense fear of rooms with flickering lights.

The laugh ot it was that the first copy of Doom I played was the PlayStation version. As it was blacklisted here until last year, PC copies were harder to come by for a kid in the 1990s, and I chanced upon a copy of the PS1 version Final Doom at a flea market, so I'm still used to playing it with a PS1 joypad :-)

Worked surprisingly well too.
It would really be nice to Project id on GOG, there's that huge id Software Pack on steam, but it honestly looks like a bit of a mess, plus there's the obviously steamworks DRM.
Nowadays I use either zDoom or Skulltag. I prefer to install both. Some mods work with zDoom only, while some are exclusive to Skulltag.

But if you have the .wad files and zDoom then it is DRM free

I grabbed that pack during the summer sale. It's a great deal. Not sure why you say its a mess. It's pretty much got every id game you'd want

Over the years of getting everything DOOM (see list below) I got it on XBOX360 as well. Great to play with gamepad.

Do I have too much DOOM?
PC
The Ultimate DOOM: Thy Flesh Consumed [original + new episode]
DOOM 2
Master Levels for DOOM 2
Final DOOM
DOOM Collector's Edition (Ultimate DOOM, DOOM 2, Final DOOM)
DOOM 3
DOOM 3: Resurrection of Evil

XBOX

DOOM 3 Resurrection of Evil (with Ultimate DOOM, DOOM 2, Master Levels for DOOM 2
)

XBOX360

I'm anticipating DOOM BFG and DOOM 4, although I'm not sure how will I get them. Any job offers here?
