Windfall for Oblivion. Not as big as Tamriel Rebuild or Nehrim, but big enough to feel like a game unto itself.

Desert Combat mod for Battlefield 1942 was incredible. It's no wonder that DICE hired the mod makers, which lead to the creation of Battlefield 2.
Of course, once BF2 was finished the team were told either move to Sweden or lose their jobs so it wasn't all good for them.

FfH2 for Civ4, my top choice was mentioned. There is also Master of Mana for FfH2 which adds new systems to the game, such as an economy for terrain improvements, and city districts, and wildlands, and.... Then there is the Xtended modmod which enhances MOM, which enhances FfH2, which enhanced Civ4 :)
And there are other excellent Civ4 mods which are markedly different games than the original. History of Three Kingdoms making a very cool use of promotions and great leaders. Civ3 also has mods which play very different than the core game. For Civ3 Ryhes and Fall comes chiefly to mind. But right now i'm playing another game of Age of Imperialism ( a scenario vs mod, but with rules change such that the AI actually builds, and uses massive naval fleets). There's more. Too many to list :)
