The design and atmosphere is great through the game, fast paced as it should be. You can feel the grimdark in every place in the game. More enemy variety would be nice, but still a great game. On Low difficulty it's still not totally forgiving, so you must be moving all time, on Exterminatus it can be a real challange. I have only one complain, sometimes you can be lost in a map, because some doors hard to find (but maybe it's my problem mostly)
