I picked up this game about two years ago along with Fallout 2 for a few bucks in a shop. Fallout is incredible - from the finely tuned combat to the story and atmosphere, which are above almost any other game I've played. The wasteland is a brutal place, and you feel like it's a fight for survival every step of the way. Fans of Fallout 3 or New Vegas will be very satisfied at how the story clicks together with the later games. The game is also amazingly replayable - I've beaten Fallout as, amongst other things, a minigun-wielding psychopath, a diplomat who beat the end boss with words and an intellectually challenged brawler (definitely a hilarious replay option - much more tasteful than it sounds, I assure you). If you, like me, came to Fallout from a background of newer games, you will struggle briefly with the controls and combat. But please stick with the game and get a handle on it - I did, and it was the greatest gaming experience of my life.
