I played the OG Mafia 1 twice back in the day... and loved every second of it. I wasn't excepting this to be a complete overhaul in every way. Graphics, cutsenes, ... pretty much everything was redone from the ground up. It all looks and sounds great. Some levels have been altered (for the better). Mafia 1 was refreshing back in the day and I always prefered it to GTA. As a game, though, it hasn't aged super well. movement/combat is clunky to say the least. The driving bits tend to get boring, although the city is a thing of beauty as you're rolling through it. To me this was more of a trip down memory lane. As a game in the GTA genre there's probably better stuff out there..
