I've also only just starting re-playing RTCW after almost 20 years! I had the same issue but downloading the most recent patch rtcw-sp-1.42d-win32-bin from this website http://www.markshan.com/knightmare/ fixed it for me. The game now starts perfectly. I'm running Windows 10 22H2.
