For some reason, the cutscenes' audio tend to stutte. It is quite annoying and I've tried configuring dosbox, but it still gives me that issue.
Do you have any programs running in the background that take up a lot of system resources such as an antivirus program or anything of the sort? Have you tried updating your sound drivers? Have you installed the game outside of Program Files and are running it as administrator?

Have you tried switching from auto core and cycles to dynamic core and a fixed cycle rate?
Edit: Thought I fixed this but it came back, never mind.

I post this for future reference of people who have this problem, because I had the same problem. After searching the internet, I found that it is a known issue that that the WC 3 video sound stutters with cycles=max setting, can be found on the dosbox compatibility page. Unfortunately, that is the setting in the GOG version.
Setting cycles to 70000 fixed the problem for me.

For me, I need to set to core=normal for non-choppy video. Unfortunately this kills the frame rate in game which requires core=dynamic for best results.

I have used the svn-daum (h t t p : // ykhwong.x-y.net/) version of Dosbox which allows you to switch between these setting with hotkeys (ctl-1 and ctl-3), so I can manually switch as required.

svn-daum also includes MUNT for MT-32 emulation, which I think is much better than the default GoG MIDI.
