I recently finished Tyranny and loved being evil and sadistic in it. Looking for more games to do similar. Been playing Neverwinter Nights 2 and enjoying it, but the filler-filled story and the lack of real role playing outside of the small choice here or there isn't engaging me that much. Will Bloodlines let me?

I already know about the patches to download and stuff. Just wanting a good RPG that lets me revel in being evil, selfish, and sadistic.

There are definitely some options in this game that can be considered morally dubious, even if you're playing a "good guy" vampire. For most of the time you can be a prick to everyone, but of course this sometimes changes quest rewards (a lot of the time this is just your "humanity" which you can get back easily enough).

What I mean to say is, to get the most out of it there are some people you probably don't want to piss off right off the bat, but there are also plenty of options to screw people over. Also plenty of options to resolve something violently, non-violently nice, or non-violently... not so nice.

Near the start of the game there are a few opportunities to get some cash using scams and blackmail, so there's that too.

There are also opportunities to seduce someone before munching on their neck, or (depending on the character chosen) control their minds, or make them go insane. Convincing people to kill themselves or giving them a mental breakdown never gets old.

You don't get to wipe out entire villages and burn them to the ground like in Tyranny though - it's smaller scale than that.

squid830 got everything right. This is a mean-spirited game set in a dark world, you only rarely ever get to be the hero. There are plenty of opportunities to bully and disregard the well being of others. Occasionally you get to break your promises and snitch on characters. Many dialogue options let you be a dick for no good reason other than to be mean. Sometimes this comes from you being a powerful vampire pushing humans and thinbloods around, sometimes it is a way for you to spite the authority of more powerful creatures.

Sounds like Bloodlines is the game for you.

Awesome stuff. Friend of mine is also recommending Fallout 2, but I'm just not a post apocalypse fan.

How about "almost apocalypse"? :)

In that case you should give Arcanum a try.
It's another great RPG where you can be an evil bastard.
It's not a post-nuclear world.
If you decide to play, I recommend that before you create any character, go here and install this unofficial patch.
Also, if I were you I'd change combat mode to turn-based. Real time is so broken it's funny.

If you're going to do Fallout, start with 1.

You can definitely be a bad guy in both 1 and 2.

Arcanum's a possiblity too, like almabrds said. Good game, cool setting.
Just blew up the warehouse. Honestly, I'm pretty bored. I hate the Masquerade. I wish I could be Sabbat.

The Ocean hotel was legitimately terrifying, though.

I agree on starting with F1 even though some of the UI is much more improved in F2, mainly due to it having a better atmosphere.

Age of Decadence is a more recent RPG I'd put in the mix. It has a lot of not-so-nice choices, and your decisions can affect the fates of large numbers of people (won't say more because spoilers). It's post-apocalypse but from when a pseudo-Roman empire collapses, except the Empire was much more advanced than the Romans in many ways.

The "End of Days Mod" (hope I go the name correct) will let you play as Sabbat.
You can be evil in FO:NV, as well.

If you mean "The Final Nights" mod by this, you can play some Sabbat clans, but the story will be the same and you cannot join the Sabbat. You can do this in the Unofficial Patch plus though to get a new ending and hopefully the next Clan Quest Mod will build on that...
