I've been trying to play both Underworld and Underworld II, but I'm having the same problem with both: everything works fine except for the cursor movement. It works well when moving the cursos sideways, but when trying to move it up or down it becomes unresponisve and badly inaccurate.

My mouse is a Razer Deathadder set up to 2400dpi.
Try reducing the dpi. You may also edit the dosbox config to adjust mouse sensitivity or try both

I've tried reducing the DPI to 100 and 500, no results.

I've also looked at the dosbox settings, but didn't find anything useful. The weird this is that left to right and right to left movement is fine, it's only the vertical movement that doesn't work.
I had the same problem with a Logitech G400s, fixed by setting polling rate from 1000 Hz to 125 Hz.
One other potential cause: unplug any joysticks, controllers, etc - anything other than your keyboard and mouse. Those have a tendency to create 'phantom' input with the UW games.
