zandrag: You didn't include the Basic settings.
Try changing the Graphic Mode under Basic to Direct3D, OpenGL, or OpenGLnb and see if it starts up.
And, I am assuming you tried the Verify/Repair under that button, Manage installatoin, Verrify/Repair?

Solstar: yes already wrote it two times i did the repair\reinstall.

it didn't get the first screenshot of basic settings for some reasons and duplicated the second one

ncarty97: I've observed that some games don't do well when installed into the default directory for Galaxy (Programs x86). Had a similar problem with Dungeon Siege trying to run some mods. I reinstalled into C:\Games and it worked fine. Might be worth a try.

Solstar: I install all gog games on F:\ which is another hard disk, and directly there, in the root, so i have f:\ultiam underworld 2
I had issues running the latest GOG versions of UU1 and UU2 on my linux system. I ended up having to shuffle around a few files to make it work. Maybe the following will help you, if you manage to adapt it to your Windows file layout:

cd __ support
cp app/GOG.BAT ../
mkdir ../cloud_saves
cd ../DOSBOX
dosbox.exe -conf ../__support/app/dosboxULTIMA1_single.conf
