It's not a bug; that behavior is by design.

You see, back in the day, computers did not have hard drives; rather, games like this were stored on floppy disks. When you want to play the game, you would insert the disk, turn on the computer (or reboot if it's already on), and the game would then (eventually) load. When done playing, you would press Q to save, wait for the drive light to go off (removing the disk while it's being accessed can damage the disk), take out the disk, and then turn off the computer (or insert another disk and reboot); there's no desktop to return to. When the DOS version was made, they kept this behavior.

If you're playing under DOSBox, when done playing (after saving), to quit just click the "x" at the corner of the DOSBox window (Alt-Enter first if using full screen); I hear pressing Alt-F4 might also work. (Alt-F4 is often given as a troll answer to questions, tricking the user into closing the active window, but in this case closing the window is what you want to do, so this is actually meant as serious advice here.)

Ultima 5 is the first game in the series whose DOS version includes an "Exit to DOS" feature; I believe it's something like Alt-Q, and I know you have to confirm this command. (For whatever reason, the command is undocumented. there's another undocumented command that may have been a debugging feature that wasn't removed; some may consider it a cheat, but all it provides is one normally hidden bit of information about the game state.)

There is a fanmade patch out there called something like the "Ultima 4 Upgrade Patch", which I believe can do this. It also allows you to add MIDI music, based off other comptuer versions of the game, to the DOS version. (Note that this is not the same as the NES version's music, which is completely different.) The usual caveats regarding game mods apply here, including the fact that such a patch can contain arbitrary code, so get it from a place you trust.
