When playing Ultima underworld I've had a problem with the way the mouse controls, it constantly stops and is just generally hard to use, is there any way to fix this?
I find it's generally easiest to use the keyboard to control movement. Getting it right with the mouse is tricky and doesn't really allow you to move while doing other things.
You may try setting "sensitivity" in dosbox's options to higher than 100. Perhaps that will help. 100 is the default I believe. If higher makes it worse, try lower. Just go to 150 for a start, you can always change it up or down.

If you have any mouse enhancing software running in the background, that could, in theory, cause some problems.

If you're on a mac, then I have no idea, i've never used one.
I have the same problem and it does not resolve with the config which comes with the game.
Anyone who resolves this gets my eternal gratitude :-)

E: The mouse movement is really sloppy and kinda glitchy, not smooth. Sometimes I find hard to click on certain things because of the mouse

A few general things:

Does your mouse have custom driver software, as is often the case with gaming mice (and their myriad buttons)? If so there might be a conflict going on there.

In the Windows mouse settings, have you tried changing the various sensitivities and enhanced pointer precision on/off?

Do you have Windows' accessibility features disabled, along with their shortcuts? (The shortcuts can be particularly obnoxious, since they often overlap with gaming controls and can "helpfully" enable things that you don't want active.)

Is it ONLY the mouse pointer or do you find you're getting keyboard control stutter, sound stutter, etc, as well?

In the Dosbox .conf file for the game, is "sensitivity=100" or some other value? What about "output="? If it's "surface" or "overlay", try changing it to "ddraw", and vice versa.

Likewise, "priority=higher, normal" should be set as such. If it's showing anything else, change it to that.
I'm using a razor mouse and was having the same exact problem. The mouse was not smooth and would just flat out stop especially if i moved in an upper left direction. I had to increase the mouse sensitivity in dosbox from 100 to 250 and it worked fine (but it was a little TOO sensitive)
