So... about the overall plot....

1. Why didn't you die when that peasant stuck a pitchfork in you way back when? Or why did you come back to life?

2. When was the exact moment you regained your memory? Was it at the end of W2 sometime or in between W2 and W3? I am leaning towards in between because I really don't remember a line where Geralt says he has his memory back in W2. Its just sort of assumed at the beginning of W3.

3. So..... what was the king of the wild hunt talking about at the end of W1? I mean, he was saying how Geralt was a way of destroying things, I guess, so..... was he interested in Geralt as a way of softening the world for the invasion of the Aen Elle? However, he definitely looks like a wraith at the end of W1 (floating, no feet, emaciated bones), so much so that it appears the devs changed the basic theory between the wild hunt between then and now.
1. In the books you didn't die. Ciri took both, Geralt and Yennefer, to some safe place where they lived happily ever after. The games are not considered the canon (According to the author of the books). It is a separate story.

2. Somewhere in between I assume.

3. No idea man. Maybe that one in W1 was just some kind of a projection, not a real person. Apart from that, I can not remember what was he mumbling about. Can't help here.
#3: You were dealing with a spectral projection of him, not the real individual. Alvin (who became the Grandmaster) was similar to Ciri in his potential but went down the wrong path to actually make use of it, hence the Hunt's interest in him.
1. Since in some of games (first?) some character (Zoltan?) says Geralt that he witnessed his death with his own eyes, games either Ciri picked up waning "essences" of Geralt and Yennifer, or it was somewhat "retconned", because where the hell bodies went?

2. Triss told she'll prepare a spell, but had no time to use it before Letho kidnapped her. So probably she did that after W2 ending.

3. First game is not greatly taken into consideration, I'm afraid.

But there is one question that troubles me. Who was the assassin from W1 outro? Letho said he only knew two his companions from W2, IIRC, but this guy was a witcher. An oversight?

1. I don't really know, I think of it as if both Yen and Geralt were "dead", but both their bodies and souls still remained in this world, and then Ciri took them to an island. If I got it right, they only woke up when the Wild Hunt came for Yen.
2. In the Witcher 2, when speaking to Letho. No doubts about it.
3. I can only guess here, but Geralt seems like one of those characters whose life is filled with some kind of missery. A lot of people dear to him die. And let us remember that he was an unexpected child, someone with a strange destiny, even if he doesn't really believe in destiny.
3b. The king of the Hunt entered in Geralt and Jaques' vision using his wraith form, that's what I think at least. The Wild Hunt only goes in their real form when they have to acomplish a very important mission, and these seem to be planned beforehand.
