I've tried max gfx, min gfx, vert sync on and off, fps at unlimtied, 30, 60 fps, pre rendered frames in nvidia pan set to 1, triple buffering on, window, borderless, fullscreen etc etc still nothing seems to fix this micro/stuttering i get when playing, i mean at this rate the dam ps4 at 30 fps was more consistent and smoother than this, i can't play with the game like this till it's fixed.

PC: Intel i5 3570k @ 4.2, GTX 970 SC, 16 gig 2400mhz memory, 144hz monitor.
Is that a G-Sync monitor ? 60 FPS on 144Hz monitor is not an optimal framerate. If it is in fact G-Sync - check the nVidia control panel for G-Sync settings. Actually you should check it anyway. Maybe some setting is messed up there. You should also disable V-sync in game and set the frame limit to 'Unlimited', otherwise G-Sync won't work properly.
Updated drivers ?
Mouse working properly ? Idk if you're playing many PC games. If you aren't, then on desktop you might not notice that you're mouse is acting up or is being used on a non-ideal surface.
I mean, you were really vague about your issue. I'm kind of shooting in the dark here.

EDIT: If you switched from PS4 maybe it's the change in controls makes it feel like the camera is stuttering. Are you using the mouse ? Maybe the sensitivity is too high. Try using a gamepad on PC maybe ? Again - not enough information.
I just started the game and took a closer look to how smooth is my camera and it kind of isn't. That's because my mouse is set to be quite sensitive. It's how I like it and I guess I'm used to the camera being a bit 'jumpy'.

I'm using gamepad, its an actual stuttering issue, there are lit hundreds of other people and youtube videos of the same problem, the fps can be 100 and it still happens, it's like the camera pan is skipping/jumping when you pan, causing a stuttering effect.
I have the exact same issue. And that's fairly recent. I played for approximately 50 without any issue, without stuttering, without any lag. Then a patch introduced the mouse delay that could be easily fixed with "hardware cursor option". But now, a more recent patch caused a LOT of stuttering.

It's unrelated to what happens to the screen and the general hardware. In fact, you can easily see it's a problem somewhere in the management of the mouse since you get the same problem in the main menu.

There's very little happening on the menu, geralt stand still, only a few embers are flying though the screen. Do nothing, everything's fine. Mouve the mouse cursor and I can see the embers lagging like crazy. In game it's the same. As long as I don't touch the mouse, everything's fine. I can run, jump, turn around with keyboard - not a lag. But if I move the mouse, lags, lags everywhere.

I tried everything. Updating drivers, messing around in game setting, in windows mouse settings, whatever.

They really need to fix the game, it's unplayble.
I double-post like a moron to highlight the fact that I found the issue. GoG overlay was the culprit. Disabling the overlay stopped the stuttering and make the game play like usual.

Funnily enough, you would expect that GoG features would works fine with game created by CDPR :)
The overlay is an 'in development' feature so I'd cut it some slack.
The overlay causes no problems for me. It just doesn't show at all ;P
Ho, sure, the client it still well advertised as a beta and having some feature in development. I'm not blaming GoG for that, it's just that I'm surprised that the first issue I encounter between Galaxy and a game happens with The Witcher 3. Considering CDPR is a sister company, I was expecting that the feature were tested more extensively on their game, that's all.

Isn't the overlay ONLY available for The Witcher 3 right now ?
I don't know for sure but that would makes the problem even weirder. If there's only one game (with the feature) to monitor, any major bug should be spotted quickly and addressed in a few days. I can't really image GoG being like "yeah, no hurry honey, we'll fix that someday"
