I started this game as a new Witcher 3 character and was never asked questions about my decisions in Witcher 2 right before I'm getting shaved. I saw on youtube a walkthrough where a guy at the same time was questioned about his Witcher 2 decisions. I then started over from one of my Witcher 2 saves and the same guy never asked me anything about Witcher 2. Do you only get those questions if you start the game without a Witcher 2 save file and choose the option that you played Witcher 2?

Yup, questions only appear if you choose to "simulate Witcher 2 save".
