How do I open the door after the elemental?
I see a shrine, but there's nothing to activate, nor is the wall ard-able.

Well, hmm. Maybe you should head back to the keep, do a bit of rummaging, some light reading perhaps.
(Around the bottom of the stairwell to the room Yennefer takes over, you should find a book, "Monstrum Volume 2" or something similar. Find it, read it, bring it with you.)
