I am trying to take out two wraiths with that customary red skull above their heads.
Before I go in for the fight I take a wraith decoction potion, thunderbolt for fast attack and
white Raffard's decoction for vitality but immediately my toxicity level bar is full.
Taking White Honey would immediately clear all toxicity but it would also immediately
stop all active potions from working.
Can anyone suggest a way around this problem?
You're taking a ton of potions, so obviously your toxicity level skyrocket. One option in vanilla is expanding alchemy skills, such as Aquired Tolerance (increase max toxicity) and Fast Metabolism (faster toxicity drop).

Or just level up some more and beat the enemy when your at an equal level *shrug*.

Yeah, come back when you're a sufficient level to take them on.
Yup.

If you want to chug potions and especially decoctions like that, you want

* Acquired Tolerance, plus gathering numerous recipes -- this increases your maximum tolerance; *and*
* Heightened Tolerance, which increases the percentage threshold of maximum tolerance above which you start taking damage.

In a pinch, you can accept short-term health loss from toxicity if you have food to balance. Doing this with decoctions is obviously dangerous since they last much longer...

Also, you shouldn't take White Raffard's Decoction before a fight; it provides some instant healing, but it doesn't provide much benefit after that healing (save for Superior level w/ 2s invulnerability, and it's only two seconds). Did you mean Full Moon, which increases your maximum vitality?

Another issue I am unsure about.
By taking, say a Griffin decoction for example, a potion which is said to provide
extra protection and other benefits, does this only work when fighting a griffin
or is the said benefits applicable against all monsters?
Decoctions always apply their benefits, although some are still situational.

Grave Hag decoction comes to mind -- it gives a small about of regeneration that increases with every kill during the battle, but this is grossly inferor to, say, Troll or Ekimmara or Ekhidna if the only enemy in the fight is a single boss monster. It's meant for fighting groups.
I have been experimenting with decoctions. Taking a wraith decoction potion on its own results in my toxicity level
jumping to almost max. If I then have to take a swallow I would then be over the limit.
I will give all decoctions a miss in future as they all increase toxicity levels.
I wish there was a potion which could cancel all toxicity poisoning effects.

Take for example, say I am facing a Golem, must I consume a golem decoction or could I use any other, say
a Ekimmara decoction? What I am trying to say is can any decoction type be used against any monster type?
Decoctions aren't for using against the enemies they're named after. That's what the blade oils are for.

Decoctions are named after enemies because you need their mutagens to make them (generally). Their effects are universal against all enemy types unless stated otherwise in the tooltip. So you don't need to drink a Wraith Decoction to fight wraiths. You need to make sure you have Specter Oil on your blades.

Edit: And they're meant for you to be highly selective in which you choose. You shouldn't be trying to slam a bunch of them. Choose 1 decoction and maybe 1 potion at low levels. Better would be to just stick to potions at first and save decoctions for when you level up some of the Alchemy skills to boost your toxicity tolerance.

For Normal difficulty, Swallow, Tawny Owl, Full Moon and the proper blade oil are pretty much all you need.
