I'm currently doing the quest for Count Reuvan that requires drinking an antidote to a potion. The Count gives me a bottle of antidote and a recipe to make more, however they do not appear in my inventory. They are listed as recently acquired, but I don't actually get them. The potion does not appear in either my potions or oils page, or my quest items page. The manuscript page is listed as recently acquired on my alchemy screen, yet it is not listed.

Basically, I can't complete the quest now.

Has anybody else encountered this problem and, if so, how do I fix it?

Clean your potion bar before that dialog.

Just gotten to this point i get the Pop's mold Antidote autoequipped to consumables, but no recipe for pop's mold antidote in equipment or alchemy. Version 1.04
I had that problem too! I also wasn't able to see the potions anywhere in the inventory!

Just reload the savegame a few times, it should work after a few tries.
And there is none. You just get 2 bottles of it that Geralt will refill while resting.
I spent some time trying to find it in my inventory. Eventually I found it in the first slot in the potion quickbar too.

Yea, potion is there, but what about recipe? After talking to Siggy it says that pops anditode recipe has been added to your alchemy recipes (or something like that, I've got polish version), but that's not true - no recipe in inventory or in alchemy book. Clearly a bug :)
not a big deal
if the potion is made, you dont need the recipe / formula again, no?

Hey guys, I know I'm late to the game but I wanted to add a potential fix for those who might have this issue in the future (since this thread came up for me when I was searching for a resolution). There might be a bug, but in my case, it was just a design/logic flaw. When you go to the Alchemy section, you have to change the filter to get the recipe to come up (for me, it was right stick on the Xbox). After I did that, the item said "You already have this item", but I didn't, it wasn't in my Potions. I used the tip above to access it through the quick menu by holding the left button (LB) and holding up on the D-Pad to rotate the quick menu. Overly difficult, but that combo worked for me.
Was looking for this one for al ong time, figured out the potion is already equipped and the recipe is "known" to me so it is hidden by default filters. Also it is not in potion sectioun of alchemy, but in Quest section below that.

Just use your quen shield and that will work also to keep u from getting effected from toxins, I did that then the potion was all of a sudden in my inventory, cause it did the same to me wasn't there u til I used quen shield then I saw and used it, even if it won't appear in Inventory for some reason the active shield and it'll let u walk threw so u can still complete quest
