Say the main quest is over and all question marks have been travelled to on the map. Are there any achievements that can't be gotten or are harder to get?

Well, there are the ones you get by finishing the game in harder difficulties.

Also, there's is the one you get if you take all possible people into Kaer Morhen. I didn't got this achievement as I killed Keira Metz when she used me... Now I'm trying to get all "?" in Skellige before going to Isle of the Mist. The game's freaking long...

Ah, I killed her as well. Didn't have to happen, but she would not listen to reason. The risk of what that research in Radovid's hands could entail was just too much. She even had the gall to call me old fashioned for not wanting to sleep with her! : /

I guess I won't worry about that achievement as my actions wouldn't allow for it anyway.
