I am on level 38 and still have not found it. I need the oil to fight an Arch Griffon.
I have looked in all the places people have suggested.
Keira at Kaer Morhen - can't find her.
Watch tower on Fyke Island.
In a cave in Velen near Hang Man's Tree.
Treasure chest in river south west of Fyke Island.
Jouna the herbalist at Rannvaig in Skellige - she is not there.

Any other suggestions please.

The only guaranteed place to get it is Keira Metz. You will only find her at Kaer Morhen at the appropriate time, and provided you didn't kill her. The appropriate time is after the Isle of Mists, when Ciri transports you there, and you meet up with those you have recruited. All other locations are random.

At Kaer Morhen the map shows an Alchemist near the main structures but I am unable to get there as a high cliff face
extends all the way around the area.

You left it too late. She's behind a locked door after the battle. There is a way up to her by climbing, but I've never done it.

To get to Kiera at Kaer Morhen, climb up the pendulum wall then jump down to the other side, make sure you roll on landing or it will take a good amount of your HP. To get out of there, find and edge where you can slide down. You won't be able to climb up the wall from that side.

I think you are right. I have the advanced and superior manuscripts but unable to use them.

The map shows a path from below the top of the wall to her but it is a vertical drop very high. I cannot believe one can survive jumping off the wall from there. (unless you are referring to some other location)

there's 3 possible way. the one I always use is the gap between the wall and the grass. When you jump make sure you hit the wall and keep pressing jump so you don't fall hard on the ground.

the other 2 options are tricky. but i did mange to get down a few times.

Even if you miss the recipes, they will show up later at other alchemists and in treasure boxes.

It always seems I start off getting one or two things up to superior in the first half the game, then can't find any others.
But then as I play the last half, they all slowly get filled in, though not in order of upgrade; I usually get enhanced, superior, then the basic one.

Success at last. I found a spot where I managed to get to the path alive. Found Keira and bought the diagram.
Hardest part was getting off the cliff without dying. I now have the Superior Hybrid oil.

Yes, you can get the Hybrid oil from Kiera early in the game but you can also get it from Jonna in the village Rannvaig on the main Skellige Island at any point in the game. I’m currently level 24 and missed the opportunity with Kiera. Jonna will be marked with an alchemist supply (the little alchemy bottle) marker on the map.

One more place where I found hybrid oil diagram was in the waters near the island south of fyke isle. Once you have gathered the hidden treasure, use your Witcher senses and take a dive in the water around the island. You will find a couple of chests in the building wreck below the water. One of these chests contains the diagram. Oh and do be careful of the drowners.
