Didn't work, crashed even quicker.

Reinstall the graphics drivers. Completely uninstall them, reboot system, install them again.

If that doesn't work either:
Do you have another graphics card available somewhere, that you could put into your PC? We'll have to narrow down the possible causes of your problem....

Easiest solution would probably be contacting official support and sending them your dxdiag-file. http://www.en.thewitcher.com/support/ (scroll all the way down and hit 'Contact Us') or contact tw2support@thewitcher.com with added dxdiag-file. Might take a while for them to answer, but at least they have (and can interpret) all the necessary data.
We still can try to figure something out, while you wait for an answer, so I'd give it a shot.

NVIDIA cards have a know issue where their onboard sound conflicts with sound cards or sound from the motherboards. This is for the 400 series cards including the 450 and 460's. I used to crash all the time in games like Mass Effect.

Many of the crashes were preceeded by the sound actually locking up.

You may want to go into the device section of your computer properties. Click on the sound devices and locate any sound devices that start with NVIDIA. Disable these. This will cause no problems because they can be enabled again at any time.

But try the game with the NVIDIA sound devices disabled. There is a decent chance that this just might be your issue because over on the NVIDIA forums it helped many people.

Tried the sound card thing, it didn't work.

I've put in a request directly on the website just have to see what they say.

The graphics card was faulty and the game pushed it over the edge. Got a replacement now its working and all i can say is the game is pretty damn EPIC!!

I got my first CTD of the game in Chapter 2, just running thru the camp on the way to the hut on the cliff. (cant finish that quest, does it have to be night time to do it?)

I'm having CTD too couple of times already. Luckily i tend to Quicksave alot. I have the increase userva to 3072 set already too.
