It seems to happen when you've taken a hit(without quen up). If you do not move after taking the hit often you will simply not be able to block or cast a sign. Roll out of the way or take a step or two, and your controls suddenly work again. Basically, I'm assuming anyway, the "knock-back state" seems to linger - you can't cast while in the animation of being knocked back, but still can't a few seconds after even though the animation has ended. Very counter-intuitive.

I've actually done some testing, basically taking hits right to the face and seeing how often and how long it took before I could put up a quen. Most of the time it either took ~3-5 seconds or so, or I just died before being able to. Also did this with blocking, blocking reacts slightly faster but there is a very noticeable delay, around 2 seconds.

Try it yourself and I bet you will experience the same - maybe not dying as much, I'm on hard difficulty early in the game ATM.

Basically, your best best after taking any hit is to roll out - trying to block or cast a quen or aard or any spells really, will often lead to your death as it simply won't work for a precious few seconds or until after you move/roll.

Very irritating, hope they manage to fix it soon.

Yes, this issue is very obvious in a certain part in Act 2 (don't want to put spoilers here). Basically even after recovering from a hit, it takes a few seconds before you can move your character around via WASD.

[Bad pun incoming] I guess that is how... Geralt rolls.
