Aye, the wraiths in the burnt out mental institution didn't one shot me, but where annoyingly difficult. Each set of 2 was an exercise in frustration. I decided to bump the difficulty down and the shit is plenty challenging.

Quen, roll, chop, roll, chop, roll, quen - rinse and repeat. Pretty much my strategy for every encounter so far.
probably need some kind of potion
Wraiths can one shot you if you let them hit you in the back (Anything striking at the back deals 200-300% more damage). And just because your facing them does not mean they can't strike you back (yep, those wraiths can face stab like a TF2 Spy.)

I beat them by trapping one with a spell, then forcing the other one into a wall, then wailing on it with sword attacks stun locking the thing until it died or its friend got free. Once it was dead, I did the same to its pal.

Thankfully, there was never more than two wraiths to deal with.
What i did was to put LOTS of traps didn't get any exp but they died lol , is really a let down to fight 2 monster that 2 shot you and if you block one to counter attack the other one will kill you the same i really don't understand.

It's not a bug, you're out of Vigor. Cast Quen instead if you want.
I actually found Yrden to be more useful, there. Before I entered the ruins I imbibed Petri's Philter (to increase sign potency) and Tawny Owl (to regen Vigor faster).

I'd toss down a Yrden and wait for one wraith to get zapped by it, then go to work on the other one. If you're fast enough you don't even have to block and can finish off the 1st before the 2nd is upon you. If you can't finish the first quickly enough, roll out of their reach and rinse & repeat (Yrden, one gets zapped, go immediately attack the other and try to kill it quickly).

Yeah, the Yrden sign is awesome against the wraiths.
Roll around, use shield move, use bombs, etc

It's not too bad on normal.
