I found that actually targeting and facing an enemy will prevent this bug most of the time. It doesn't make sense with Quen, but it appears to work for me. Definitely an annoying bug though.
I think this very problem is the sole reason why EVERYONE complained about combat being too difficult at the start of the game on May 17th. We just couldn't get the proper "feel" for the clunkiness. Now that we've all subconsciously factored in this handicap into our combat technique, we call all get through most fights without a problem. LOL

I think it is certainly a big factor. Agreed.

Geralt now spins and jumps like a crazy man. He even targets what he wants to hit without locking on to it. Switches between targets to keep them staggered. I don't know how he does it, 'cause he sure couldn't when I first met him in TW2. :P

And then he throws a couple bombs and locks up, or stares off into space instead of casting Aard, and I am like, o yeah. Not fixed yet.
I know for a fact that you can't cast the medallion immediately after letting go of a directional key.

Example - Holding W to run forward, let go of W and then immediately press Z. The medallion will not cast. You have to continue holding W and press Z to immediately cast the medallion.

When you release a direction, Geralt enters a stopping animation and during this animation the medallion is unable to be casted.

Something like this could very well affect other actions.

Agreed. I was completely disgusted on day1. I finally booted it back up a few days ago, determined to get through the prologue, even though the OTS cam still bugs me. Thats when I noticed everything being described in this thread. It seems like the controls are about the same response wise as witcher 1. Problem is, the combat is far quicker paced than that of witcher 1.

Its immensely annoying, and I have resulted to spamming buttons. The same that happens with signs also happens with traps and bombs. just does not respond to keystrokes / clicks.

Hopefully someone's posted this on the official forums. That would explain why my sign casting is so temperamental.

That needs to be fixed. It's gotten me killed often.
