Which of my decisions in this game carry over into the next one? Is it just major story events, or do the side-quests matter too?

They are listed on Witcher wiki (beware spoilers from the The Witcher 1 & 2 if you haven't finished them).
Most of them are from main quests but there are a few from optional ones. It's mostly about who you side with and whether you kill certain characters or let them live. Romance choice has a minimal impact (literally just a journal entry). You can also carry over some orens and swords/armor (but they will be a lot less powerful in The Witcher 2).
