No one mentioned talking to the "hermit" in the fields about the "Paths of Destiny" and the Holy Grail. That also involves the Wild Hunt and some additional quest(s) from the Lady of the Lake. hint:. When asked if you wish to pursue the grail -- say no! You don't want to get her mad.

I was wondering the same thing... if some of the problems listed above were because of lack of interacting between Lady and hermit through Geralt.... besides doesn't the Grail question line lead to her picture card. :)
Sounds like a bug or something. When I was knighted by the lady of the lake, berengar showed up near the boat. But before that I mostly always completed secondary quests. And talked too many same sentences with everybody. =)
And the gift from lady of the lake... I't was the sword right? Or the sex. Didn't notice any particular talents.
Well, I just assumed it was scripted that berengar automaticly comes there after the sword thing.

The Lady of the Lake gives you a talent if you solve the Paths of Destiny quest in a certain way. Annoyingly, she tells you to "come back later" for it so you have to leave Black Tern Island and then return again to get the reward. And you have to do this BEFORE tackling Dagon.

The OP seemed to have an issue with this because he never got the reward and therefore the Paths of Destiny quest was never officially solved. My guess is that this is what was preventing Berengar from appearing, but it's possible there was a different cause.

I don't know what's the deal with original poster but many times people said that come back tomorrow.. or "i'll work through the night" and that dwarf was sleeping when I came about 2 hours later to get my armor. =) And some sidequests were a bit vague. They would be there the whole game. And V chapter.. no storage. That's why I didn't complete the dentist thing. One freaking tooth. And cockatrice contract failed at some point. Didn't even see them anywhere.
But I did everything else I could find.
Main thing is still that I played the game.
And when the credits rolled.. just when the music was getting good, then came main screen. Anyway.. Game was great. =P

It was devourers teeth.. I had those in my storage but didn't have access to that. And there wasn't any of those (devourers) in chap V. But life goes on.

For future reference, they're on Black Tern Island. When you arrive on the island, keep to the left as you go past the Lady of the Lake. There's a bit of beach over on the left side there where the cockatrices are.

No, those are the basilisks. Julian has a contract for their hides in Act IV.

Yes. Only creatures that island has (I remember) is wyverns and basilisks. And the cokcatrice thing failed..well I noticed the x in journal in chapter four. So I should have done that before. But no big deal.

Edit: The game didn't exatly shout if some non essential sidequest fails. =)

It may lead to her "card," There are refences to her knights (one who is still living) which can also be useful in helping you decide the "path of destiny" and your decisions which at the end of Chapter 4 can effect the entire outcome of the game. I believe Chapter 4 is very sad but it is filled with much soul-searching for Geralt ie: feeding elves and the like. You may have to slow down your quest progress to repeat some the encounters with NPCs with which you have already had discussions. Then back to Berengar.
Help I have this same exact problem and have been stuck in chapter IV for 3 days real time. I can not beat the game like this. I try going back many saves and have been trying to replay the chapter farther and farther back but no luck. I do not know what is wrong. Seems like the game is broken. So far I have been playing for 17 hours trying dozens of saves but no luck.
After spending 2 days of finding out the issue.
I tried to find out the glichted quest.
i did manage to edit all the quest then is find out gliched quest is Alvin(It gliches if you skip somehow any of the talk in VIllage and Lakeside)
If anyone still have this issue, Please try this, Alvin should talk to you twice(Village and Lakeside) to get it right also you have go to left side in lady of the lake to find Berenger then all will be fine where find some hides for quest.
