Looks like it's a mod problem, Last Resort mod borks the framerate for some reason, which is a shame since I use it to support higher resolutions.
Screw it. Massive slowdown even without the mod now.

I'm either going to try Virtualbox or my old XP laptop.

Is there a list of games this affects?

This is a mostly wrong statement. The mod only has a very slight affect on the frame rate and is a Windows issue. You can get the same problem with the 2D menus even when the mod is turned off or isn't even installed. Yes, if you have a stutter issue in the 2D menus you will get a slightly better frame rate if you only choose 800x600 pixel 2D menus while using the Last Resort mod. The stutter only affects the 2D menu system of the game. You can still set your 3D setting to whatever you wish without issue.

The problem is with Windows 7 and beyond. Windows 8 makes it even worse I believe due to the fact that 16bit colour mode is emulated and it has terrible support for old Direct Draw games.

Unzip the file and drop ddraw.dll, libwine.dll, wined3d.dll into the SWAT3 folder.

Now tried on a AMD desktop, i get fps drops. My nvidia computer got no issues at all but it also got gtx 970. If you buy the game at this stage demand money back asap as you should not have to do strange 3rd party solutions to play without fps drops.

dgVoodoo 2 wrapper doesn't work for a lot of people. Did you do anything special to get it to work?
Unpack dgVoodoo2_45.zip Transferred the files from the MS directory (D3DImm.dll and DDraw.dll)  to the directory with the game.
Ah discovered what the problem was with dgVoodoo.

If you choose a higher resolution than the old SVGA resolutions you will get an error about not being able to set 2D or 3D modes.

The way to fix this is run Swat 3 with the Last Resort Launcher and change both the 2D and 3D to 800x600 resolution. Run the game and test out the 3D in one of the maps.

You can then quit Swat 3 and then the next time you run the Last Resort Launcher you can choose any resolution you want. You only have to apply this fix once (unless somehow your swat.cfg gets mangled with a Swat 3 crash).

Tried the WineD3D files, couldn't get it to start at all, tried various resolution gimmicks, utterly stuck at below 10 fps on a Win10 prof. machine with an AMD :\

Did you try dgVoodoo? I had problems with WineD3D on an AMD computer on Windows 8, since then went to Windows 10 and dgVoodoo and works.
For me, dgVoodoo2 solved the framerate problems with Windows 10.
