Try deleting the swat4.ini file inside \SWAT 4\Content\System\ and tell me if that worked.
Just to note, I get the same issue too:

This happens every time I try to load the 3rd mission (QuikFuel). I have deleted my ini file but the same issue occurs.
Same here, I'm getting this error when trying to launch the red library mission (it's the fourth or the fifth, I think). Is there really no general fix to this problem? :/

(Deleting my ini file didn't work).
Same here, i bought today and istalled not working!! can admins do something?

i tried delete ini files. doesnt help any.

and always when i try to start it makes automatically new swat4.ini file.... is that normal?

I just bought the SWAT4 package myself, and I have the same problem as mentioned above. Any solution? By the way, I use Windows 10.

Venita: Try deleting the swat4.ini file inside \SWAT 4\Content\System\ and tell me if that worked.
Just want to leave a positive solution experience. I had another error on the same mission (just a coincidence, I suppose): "IDirect3DVertexBuffer9::Lock failed(88760870)." Renaming the .ini file worked for me. Seems that some graphical setting lead to the error.
if someone has a critical error at the start the problem can also be through the second monitor. When my friend disconnected the second monitor in the controller, Swat 4 started normally.
i just bought and downloaded swat 4 and it keeps giving me the general protection fault error. Anyone know how to fix this?????
This maybe a little late but what worked for me is opening the properties of the swat 4 shortcut and changing the target to this: "C:\GOG Games\SWAT 4\Content\System\Swat4.exe" -nointro -windowed

and this for the expansion: "C:\GOG Games\SWAT 4\ContentExpansion\System\Swat4X.exe" -nointro -windowed

All this does is add the "-nointro" and "-windowed" to the end windowed doesn't work but I found this on a website were that was included in the end. This is the website induktio.net/wiki/Command_line_parameters.

I don't know if this will work for you but the intro seems to be the reason the error pops up.
