As much as I love GOG, I will say that the Steam version of Surgeon Simulator 2013 is DRM-free, so if you want to buy it through there, you can launch it without Steam AND have the Team Fortress 2 content.
Steam itself is DRM.

what he meant by that was TF2 and their characters are owned by Valve so rarely will you see Valve allowing TF2 characters in non Steam versions of games since they are often there as a bonus for the Steam versions of a game. The only exception I am aware of is Hero Academy where I could use the TF2 skins when playing on the Android version of the game but that is probably due to how the game works i.e. you can start a game on the steam version and play your turns on the go in the android/ios versions.
