I'm having a hard time figuring out the difference between Stronghold Crusader HD and Stronghold Crusader Extreme HD versions.
So the Extreme one seems to have higher unit cap, some special powers and a new campaign.
I never played Crusader before only the first Stronghold (before it went HD).

What I'd like to know is, is everything that Stronghold Crusader HD have also in Extreme version and can I completely ignore the "vanilla" version and just play Extreme from start. Is there any difference in difficulty?

Depends, I personally would recommend the HD version over the HD Extreme version, especially when you haven't played Stronghold before. Extreme was designed to give old players the chance to replay the game in a more arcade like style with a huge unit cap. A lot of people find it unbalanced, though. I'd go with the standard HD version as it gives you the original Stronghold feeling that made the game a success. Decide for yourself later if you want to go berserk by spawning tons of units.
Play Stronghold Crusader HD first. The "Extreme" version feels extremely unrefined and the novelty wears off quickly. It's worth trying at least once, but definitely play the original first.
So the Extreme one is something like stand alone expansion, right?!
Most strategy games are not made for high unit caps, so balance issues are what is to be expected. However if you like to play games like Starcraft or Age of Empires with very high unit caps – which, in my opinion, kills the game –, then you can give this a try.

The unit caps actually aren't directly an issue. They were so high in Stronghold Crusader as to be effectively impossible to reach in anything other than sandbox mode. The problem is that Extreme just gives you massive numbers of units (more than you could ever possibly build yourself) without requiring you to have any particular economy built up. It's only by virtue of these free units that you could possibly make use of the expanded unit cap. This completely devalues the game's economic side and robs a lot of the game's depth and charm.
