Installer only.

GOG isn't exactly strict with that definition even when they use it. E.G. Arcanum has a changelog:
Internal Update (30 March 2018)

[WIN] updated internal installer structure, no changes to game files
The new installer actually has a different main EXE (UPX compressed) than the old installer. It's believed to be functionally equivalent though.
