While playing, my game screen will flicker, showing me the desktop on that monitor and my google fu is failing to find a solution.

I have multiple monitors but only play on one if that matters.

RoM1? Try to replace ddraw.dll with this one: google "DDrawCompat" and download v0.1.1 (last release was bad for me, cuz of too much flickering cursor. well, you could try different versions)
Btw, it is the only solution that unlocks 60 fps (at least for me).
