Are the servers for Q3A still up? If they are, do they use GOG's version of Q3A or will a mod have to be installed?

Yes there are about ~200 servers up and You don't need to install mods because if You connect to any server with mod installed you simply download it and then play (remember to turn on "autodownloading" in options).
