Whenever I'm running around, the view will randomly jerk completely up, down, or behind. It's impossible to play as quite often when I'm shooting something the view will do a 180 and in the blink of an eye I'm shooting the wall behind me. I've attached a screenshot showing the compatibility settings I've tried. Neither change anything and I don't know what else to do. I never got to play Quake in the day so I'd really like to be able to play it.

What is the exact filename you tried to run with compatibility settings (is it really quake.exe)? Have you tried to move/look around using your mouse or gamepad/joystick, if yes which of these are connected? Try to disconnect one of devices. Though it never happened to me and i play Quakespasm using both mouse and DS4 connected (3rd party tools disabled).

Also you haven't stated which OS version of Quakespasm you're running (e.g. Quakespasm 0.93 (64bit) on Win7).

Did you rename the exe? That might cause Windows to use wrong (i.e. vanilla Quake's) compatibility options.
Make sure you are using the latest version of Quakespasm (0.93 or later), and also that you are using the SDL 2 version (quakespasm.exe). NOT the SDL 1.2 version (quakespasm-sdl12.exe). SDL 1.2 has known mouse issues in Windows 10.

Thank you so much, this fixed it for me. I was using the SDL 1.2 version all the time. It would randomly jerk the view upwards every minute or so. I even went as far as replacing my mousepad and replacing the batteries of my mouse because I couldn't figure out what's wrong.
With SDL 1.2 all mouse can works properly only with a polling rate of 125MHZ
