Right click on the QuakeSpasm executable file and click on "Create shortcut", then right click on the shortcut and click on "Properties" to add the required parameters to the command line.

Or follow this guide.

In step 3(install guide) it says "Important - AD 1.50 must be installed FIRST!" -How do I install it?

And then it says
"Open and extract the contents of the file 'ad_v1_50final.zip' into the directory. The 'Forgotten Sepulcher' is not a standalone release and requires the previous version to work."

I dont have any file like that!(I assume its v1.70 now)

I also tried the shortcut u mentioned. How do I add required parameters in Properties?

Thanks.
Actually, you don't need to create a shortcut for every nex expansion that you download, all that you need is to have a folder in your Quakespasm directory for each expansion that you download (for example, the folder ad for Arcane Dimensions) then just launch Quakespasm, bring the console and type: game "name of the expansion folder" then press enter and play whatever expansion you want, for example launch Quakespasm then bring the console and type "game ad" and press enter for playing Arcane Dimensions. ;-)

This method will not work for many mods requiring other mods (e.g. Quoth). At best they'll run with lots of errors, most probably they'll shut down with host error message. But you may try it for yourself, for instance download Warp Spasm mod and run console command "game warp".
Just so happens that SimonOC also uses Quoth models for his Arcane Dimensions mod but somehow he managed to get the approval from original creators to include them thus removing the requirement for it.
As for the shortcuts, some people myself included prefer to create a shortcut for exact mod just once and not running Quake and then typing what mod to play all the time.

Running Quoth, Hipnotic, Rogue mods with simple console command "game ..." may also bring minor issues on some engines, but latest Quakespasm should have forced support for them.

Proper shortcuts for original Quake or other engines with limited support are:
-hipnotic -game quoth
-hipnotic -game hipnotic
-rogue -game rogue
