Quick question : during my first playthrough I unlocked that "trapdoor" that acts as Master Staircase for the Endless Paths, but on my current playthrough it just stays locked and I can't interact with it and I can't remember how and when it happened the first time. At first I thought it'd unlock with a specific stronghold improvement but now I got all of them and nothing happened. I found the staircases at levels 4, 8 and 12 (I think that's where they are) but they always bring me back only to level 1, so I then have to manually go through that level and the dungeons to actually exit the Paths and it's a wee bit annoying since you're not really supposed to do that dungeon in one go.

Have you exited the Endless Paths level 1 through the trapdoor? It sounds like you are always leaving through the dungeon. IIRC the trapdoor becomes accessible from the courtyard and the map after you have left the Paths through it for the first time.

In the northwest of level 1 there is a ladder on the palm of the huge adra statue. That is where the trapdoor exit should be.

D'uh, so obvious now that you mention it. Worst part is that I did check the adra hand while exploring the level 1 of the Paths way back when doing the Maerwald quest but stopped just before I could see the stairs. Now I remember when I actually found those stairs in the first playthrough : after the Dragon gave me the amulet and I had to bring it there. So still extremely late actually.
