Okay, I've got a weird issue that I don't quite know what to do about. When I'm in the main menu, I can click around, select options, the whole nine yards like normal. But as soon as I load a level, I can move the mouse and look around, but I can't shoot. Can't spin up the Pain and I can't fire the Killer. Well, I can fire the Killer, but only by using the CTRL key. I thought this might be a side effect of using one of the Mega Update or Unoffical Update patches, but even just running vanilla, I run into the same issue. When I hit escape to exit the game, the mouse cursor will be there, I can move it around, but again, I can't click on anything.

I literally have no idea why it's doing this or how to fix it. I've attached my DxDiag here. [url=]http://dumptext.com/qcDWgEOl[/url]

Short hand for my specs are as follows:
Windows 10 64x
AMD FX-8350
NVIDIA GeForce GTX 1080
16 gigs of RAM
And I'm running two monitors.

Anyone got any suggestions or solutions?

I know this is years late, but I had the same issue and unfortunately the only fix I could find was a work around I developed myself.

Here is the same thing I posted on Steam:

"Okay so my work around works good enough:
Download this program:
https://www.highrez.co.uk/downloads/XMouseButtonControl.htm

Then run the program, click the "ADD" button to make a new profile and select the painkiller.exe so it only uses that profile while the game is running.

For the right and left mouse buttons select the "simulated keys" options and the "during" option and then select 2 keyboard keys you wish to use. I used J and K.

UNSELECT the "block original mouse input" checkbox as you still need the original left/right mouse clicks for the main menu itself.

Then in the painkiller controls section change fire and alt fire to those keyboard keys respectively.
Play the game!

When you need to do something in the main menu you need to alt-tab out and back in, and then the mouse will work in the menu so you can save your game, quit, etc.

You will of course need to launch the X-Mouse Buttons app and ensure it is running before starting playing Painkiller."
