I had a similar problem on Classic UT from GOG as well.
Every time I play it's like running through treacle. The FPS are fine but it just moves in slow motion and it's very irritating.
I tried setting the processor affinity to single core for the Painkiller.exe but it only barely helped and carried on 'treacling' in the presence of enemies.
Any ideas would be much appreciated!

Seems someone else had the same problem with UT. He solved it by lowering processor speed to 50%.

No, it doesn't seem to change much. I thought it had improved at first but then it went back to Treacle-vision.
This is a known issue with Painkiller that can have one of several fixes.
If you've already tried processor affinity / speed, try disabling character shadows in the video settings in the game, as this is really the main chokepoint for the engine.
Also check if you have processor scheduling set to "Programs" or "Background Services". If it's the latter, set it back to "Programs", then reboot and try the game. (Found in Control Panel -> System -> Advanced -> Performance Settings -> Advanced).
If you have an AMD processor, try downloading the AMD Dual Core Optimizer from the AMD site and using that.

Nope, still no progress. Disabling character shadows seemed to make it a bit better but it still treacle-ifies after a few minutes or whenever there are fog and particle effects on screen (even with particle effects at the lowest setting, etc.).
I'll try opening a support ticket.
I think I remember Painkiller doing that if the system is simply too slow. It wouldn't drop frames, but simply play in slowmotion. Try reducing all your settings to minimum and see if it helps. Then reenable the features one by one and see when the game starts slowing down again... also, try updating your video drivers. And make sure your gfx card doesn't overheat.

Well, I am getting slow down even though my computer far surpasses the requirements and all the settings are set to low. As far as I can tell, the game engine is poorly designed and optimized.

I had this exact same problem with Painkiller Gold which I bought retail. IIRC, the problem went away when I patched the game, but since the GOG version should be patched already, I guess that's not a big help.
I should mention that I also have the GOG version (just played it) and I don't have the issue on that.
I seem to have this issue as well.
I grabbed the demo from Steam, and it played excellently, so I figured I'd buy the full version. Sadly, this is rather unplayable.
This happened to me when I first tried to play it. For a while I thought thats just how the game was. I recently loaded it up again after around 4 months and lo and behold the problem had resolved itself. Strange I know. So there you go, wait a couple months and the game will run like normal!
Are you playing on a netbook? And if so, is it overheating? If so...That was my problem. And I fixed that fast!
