I've installed the game recently (in its .18970 version) and something is odd... The shortcut created on my desktop leads to a program called "Outcast 1.1.exe". When I run it, a window appears showing the Outcast box art, but then nothing else happens. There are no error messages or anything, the game simply doesn't run.
However, I tried running the program "OC1.1.exe" instead. It seems to run the game properly, except that the intro cinematic doesn't run and the game skips straight to the scene with the protagonist lying in a bed.
I haven't played the game beyond that so I don't know if it works properly or not, but I suspect this isn't what's supposed to happen. Is it just a problem on my end? (I'm using Windows 7 btw)
It's been a while since i ran this, but I didn't have any problems. I think the window showing the box art is actually a launcher where you can change graphics settings and controls before starting the game.

I don't remember exactly what I did to get it working, but I did write a post about the game on my personal blog a year ago which covers some of the setup stuff, if that's helpful:
