I have had 2 items disappear from my inventory. The Silver Chalice of Moonbow which I'm suspecting didn't survive the move from Chapter one to two. And Neurik's Silver Charm. I had 4, have saved the 3 boys and used 3 and JUST defeated Sir Karathis when I discovered that my last one has disappeared. I found an old hack from 2002 to respawn the Silver Charm into the pool but that didn't work. Anyone have any idea how to get these 2 items back?
I believe the Silver Chalice of Moonbow is a quest item that is only useful in Chapter 1, so it was removed from your inventory once you finished that chapter.

It has been too long since I've played the quest with Sir Karathis, but if the fight has stopped, then I'd expect you gave him the charm. If you haven't given him the charm, then try spawning it via the console.

Looks like the item code for the chalice is "nw_hen_lin1qt".
I'm guessing the item code for the silver charm is "m2q1silvercharm".

I'm suspecting you are right about the chalice. Console doesn't recognise nw_hen_lin1qt as a legitimate item code so, as you say, it just doesn't exist in Chpt2.
I tried the silvercharm that you suggested. Console says "Success" but it doesn't appear anywhere.

After I defeat Sir Karathis I end up during the conversation with only the "prepare to die" option or one that says "wait here and I'll be back" wherein he replies "with an army or a charm?"

Any other thoughts?

I must confess to being a TOTAL newbie to this. I started playing D&D when it was 3 books and some crappy dice in a box. I'm now in a position where I have a lot of time to kill sometimes and decided to "re-enact my mis-spent youth" I'm having a blast but it seems that there is a lot of assumption within game designers that players know things. I'm getting there but it is an "interesting" learning curve.

Thanks for your help. BTW....where does one acquire a list of item codes? I've seen ones for weapons and armour and suchlike but nothing for these esoteric items.

I'm honestly not sure where the item is supposed to spawn. I'd look through your inventory, move your character and see if it spawned beneath your feet, and look around the area where the mouse cursor happened to be when you finished entering the code.

"At your feet" would be my first guess. If it is there, you'll probably need to move off it before you can select it.

gamefaqs.com is my typical go-to site for guides and lists like this. In this particular case, I checked this faq.
I tried it again, looking everywhere. I moved, I looked in the pool, nothing. I tried spawning a book (thanks for the link, BTW) that I know to be in every chapter. Same thing. I now have this vision in my head of somewhere in the NWN world silver charms are falling from the sky and piling up. Who knows....maybe I'll find one on the road and be able to complete the side quest.

Items spawn at your mouse pointer location. Place it right in front of your character and then type in a spawning sequence.

Yep, Kerebron is correct. Items spawned using the dm_spawnitem console command spawn at the location of your mouse cursor (this can sometimes spawn the item in inaccessible spots like in the roof :P)

The various henchmen quest items like that Silver Chalice have no purpose outside of the chapter they're found in, so I highly recommend rotating through your henchmen every level or so to talk to them about their backstory so you get the quest and are able to deliver the items. Basically, the special item you receive from each of the henchmen for completing their quest is the trigger item to start their next quest in the following chapter.
I can confirm that this works: i've lost exactly the same object from my inventory (did I dropped it anywhere else?... I've no idea), and used:

##DebugMode 1
##dm_spawnitem m2q1silvercharm, while the mouse cursor is in a valid place to spawn the object
