I was able to get keyboard controls working in OS X on a desktop Mac using the "fn" key to access the Function keys. FN + F10 got me to the Settings menu, then the arrow keys worked to get me down to "Player 1 Keys," hit enter, and it asked me to enter in keys for each move (high punch/low kick/block/etc). This is how I had it set up back in the day in DOS, so I didn't try any other input method (yet).

F1 (fn+F1) then started the game for me..

I think the key is knowing the F keys and to use "fn" on mac keyboards to access the F key functions.
