All lewd scenes are censored with a mosaic, even applying the patch from the developer's website for the applicable game doesn't uncensor them.
The Denpasoft version is fully uncensored with no mosaic, which you can see in the screenshots on their site.

Can be UNCENSORED with patch.
Go to their official website (sushi-soft dot com/) and click on the game you are looking for.
There are 2 buttons: one to download the patch, the other for instructions.
It is easy.

According to the OP even using the patch from the developer does not uncensor it ... so now I am confused.

Edit: Just opened the R18 patch from sushi_soft with an asset viewer and can confirm that it has mosaics. It does not seem that there is any patch available which would uncensor the GOG version completely. You'd have to buy on other stores.
