I've seen it is on the broken achievements list I wonder though whether anybody was successful as GOG shows 0.1 % rate.

Have made two playthroughs one on Very Hard (have started it before first patch) and one on Hard. Unlocked everything besides "Destiny Dominated" on first playthrough, have unlocked it on second one, no Perfectionist though.

A little late, but I'm pretty sure that Perfectionist is bugged on Galaxy. I got everything and it never unlocked for me, I put in a support request and they were able to unlock it for my account. No idea if the problem is unique to Galaxy but that is the state of it.
